Steps for Admitted Students
- Passports and Visas
After having been accepted for admission to Mayville State University, students will receive a letter notifying them of their acceptance. Students will receive the Form SEVIS I-20 Certificate of Eligibility for non-immigrant "F" student status or Form DS2019 for sponsored student "J" status. In addition to obtaining a passport, a student must also procure a visa from a United States Consulate. This visa stipulates that the student is coming to the United States for the purpose of study and is expected to return to his/her respective country after completion of the academic program.
- Vaccination Records
All students must bring with them original or certified copies of their records of vaccinations for communicable disease (diphtheria, measles, rubella, whooping cough, tetanus, polio) in order to be allowed to register. For those students who have lived within the past 5 years in a country where tuberculosis (TB) is endemic, documentation is required of TB screening within 6 months prior to college entrance. Questions concerning this requirement should be directed to the Student Affairs Office, Mayville State University, 330 3rd Street NE, Mayville, ND 58257, USA. TB screening must be completed upon arrival on campus. You will not be permitted to register for classes until you show documentation of TB screening.
